Please contact the office for verification of your insurance benefits. We are out of network for all insurances, but will submit your claim for deductible credit. We use outside laboratories to process lab test and will include your insurance information for them to bill your insurance. If you are not covered by health insurance, the lab will bill you directly. Refer to FAQ
